Tuition is due monthly at the first rehearsal. A late fee of $10.00 will be added if tuition is not paid before the 2nd rehearsal of the month.

I for myself and/or my minor child release and forever discharge and agree to hold harmless PopStar Performers and any of its subcontractors for any liability, claims or demands for personal injuries, sickness, or death, as well as for expenses due to loss or damage to personal property and lost wages. Since dance is a physical activity, injuries may occur. Each student/guardian has the right to decline participation in any activity which they are not comfortable with or which they feel may be harmful. Each student/guardian is fully responsible for any injuries or harm occurring before, during or after a class with PopStar Performers. Student/guardian is also responsible for informing instructor(s) of any physical limitation which may prevent full participation in rehearsals. It is required that all students be covered by their family’s insurance. If injury occurs, it is understood that the student’s own policy is the only source of reimbursement. Additionally, PopStar Performers has permission to use photos/videos taken of the above signed student for any advertising and promotions.
Please describe your students comfort level in front of an audience. Would they be considered shy or the star of the show? What is your students vocal and dance ability? Strong ability, Intermediate level or still learning?
